Lung abscess is most frequently caused by which mechanism?

Aspiration

Lung abscess most often results from aspiration of oropharyngeal contents containing mixed oral bacteria, especially anaerobes. When protective reflexes are impaired—such as with alcohol intoxication, anesthesia, stroke, seizures, or poor dentition—these bacteria are inhaled into dependent parts of the lung. The infection evokes intense inflammation and tissue necrosis, leading to cavity formation that often contains pus and may show an air-fluid level. The right lung is more commonly affected due to the right main bronchus being wider and more vertical, making aspiration more likely. This mechanism distinguishes abscess formation from viral co-infection, which typically causes non-necrotizing pneumonia; pulmonary edema, which is fluid overload in the interstitium/alveoli; and pneumothorax, which is air in the pleural space without cavitating infection.
